Filling out a referral for a preventive health check-up
The employer must correctly and accurately complete the referral form, which is the basis for carrying out the appropriate examination. The referral form contains information about the worker, the workplace, the risk assessment and the specific health requirements determined by the occupational medicine specialist based on the risk assessment for the individual workplace. The referral can be for a preliminary examination (before starting work) or for a periodic/targeted examination (at certain time intervals or as needed). ( Link to referral form )

Organizing an appointment for an examination
The employer makes an appointment with the occupational medicine clinic for the examination. At our center, we offer the following ordering methods - electronically, via email or by phone call.
Employee notification
If you provide the appropriate contact information (email address) when booking an appointment, the latter will be automatically notified via email; they will also be sent instructions before the examination.
Completion of the hearing and issuance of documents
When the occupational medicine doctor receives all the required information (the subject has completed all the tests and submitted all the documents), he/she completes the examination. He/she provides the subject with a medical report and the employer with a medical certificate. A medical certificate with an assessment of the employee's work capacity, which contains only information about whether the employee is capable of working in a specific job, without medical data. The employer receives the document in electronic, encrypted form to the agreed email address, which ensures the security of personal data.
